Individualized Program Planning

Service Description

Discussion of program that includes ways to maintain and strengthen function in areas of concern and promote health and safety at home for individuals facing decline and their caregivers/loved ones. Program may be designed and shared at one time, or come with practitioner sessions, as per need. Program activities may incorporate respite time for caregiving family members while practitioner works with their loved one on skill-building tasks.
